OVERVIEW: The Medical Watch Program is a free program designed for seniors and individuals with a medical condition to provide pertinent medical information to emergency personnel who respond to and treat home emergencies. This program contains much of the medical history needed by the responding emergency personnel, such as existing medical conditions, allergies, medication currently being taken, and emergency contact information. |
HOW IT WORKS: Medical Watch participants complete an Emergency Contact and Medical Information form which is placed into a database at the Sheriff's Office. A "Medical Watch" sticker is placed on the Front door of the residence in plain view, to alert emergency personnel that medical information is located at the Sheriff's Office. In many cases, this information could prove invaluable to the responders. |
HOW TO APPLY: The Medical Watch documents may be obtained by contacting the Jackson County County Sheriff's Office at (850) 482-9624 or by sending an email to Medical Watch Inquiry. |
EXISTING MEMBERS: Existing program members can click Medical Watch Information Form to print out additional Medical Watch Information forms. ** Note: You will need the free Adobe Reader program to view this form. |
